Spring View Medical Centre has been offering patients the opportunity to

  • book your appointment
  • request your repeat prescriptions
  • have access to any allergies, medications and adverse reactions that are recorded on your medical records.

If you wish to sign up for this service, you will find the relevant registration form and patient information leaflet below

We are now extending the service for patients to be able to view medical records in 'Coded Detailed' form using the same system 'Patient Access'

Spring View Medical Centre are working towards allowing their patients to have on-line access to the 'detail coded' information from their medical record. It is our intention to have this development available to our patients by 31st March 2016.

Statement Intent and Plan-1516

From April 2016 our patients can now apply to have access to their 'Detailed Coded Records'.

Please ask at reception for more information or an application form. Any patient aged 13 or above is eligible to request access to their coded records.  Once you have gained access and you have any queries, you will need to book a telephone consultation with the GP as our receptionists will not be able to answer any queries about your medical records.

If you want access, you will need to provide proof of identification but this is all explained in the leaflets and application forms

The practice has the right to remove online access to services for anyone who doesn’t use them responsibly.
