Are You between 40-74yrs?

Everyone is at risk of developing Heart Disease, Stroke, Type 2 Diabetes or even Kidney Disease but these diseases can often be prevented.

Primary Care in Bolton have been running a programme via the Bolton Quality Contract to identify patients as being at 'High Risk' through having an NHS Health Check. NHS Health Checks are aimed at people between the ages of 40-74 years that do not already have a history of CHD, Stroke, PVD, Familial Hypercholestrolaemia and Diabetes.

Those patients that do not suffer from the above should have an NHS health check every 5 years.

Please contact reception to make an appointment with our Health Improvement Practitioner, Practice Nurse or HCA (Help us, Help you!) - NHS Health Check

The practice has a health trainer on site every Monday. The health trainer can support our patients in making healthy lifestyle changes.
